Application Process
Maximum of 3 pages for the Grant Application
Deadlines for grant submission are on the last business day of each month by 4:00 pm.

One copy of the Gary E. Milgard Family Foundation Application
and the following attachments must be submitted.

IRS letter evidencing tax exempt status. |
 |
List of members of Board of Directors or other governing body. |
 |
Most recent audited financial statement. |
 |
Current unaudited financial statement and balance sheet. |
Organizations will be notified in writing of the Foundation's decision.
Recipients of the Gary E. Milgard Family Foundation Grants must submit a
Grant Report Form
to the Gary E. Milgard Family Foundation within six months of the use of the funds
or completion of the project. For capital campaigns, status updates should be submitted
every six months until the campaign is completed. The report must fully describe the use
of the funds and must provide an evaluation of success. Failure to comply with this
requirement will disqualify the organization for future grants.
